All students are required to have the following forms on file with the office prior to their first day:
Registration/Enrollment Form
	Greenbelt Children's Center Enrollment Agreement
Health Forms (To be filled out by your child's doctor.)
	Health Inventory
Other Forms
	Emergency Information Form Family Information Form Confidentiality and Health Insurance Information Form Parent's Guide to Regulated Care (please tear off to sign and return stub)

Your emergency phone numbers are vital to your child's safety. Please be certain to keep the Center informed of all changes of phone numbers. It is also very important that all addresses are kept current. Please notify us as soon as possible when information pertinent to the emergency card has changed (including phone numbers for those persons listed for contact in an emergency). Emergency information cards must be updated and initialed by parents annually.
